Welcome to Ashgrove Medical Practice
Whether you have just moved to the area, are unhappy with your current GP, or have never been registered with a doctor before, we welcome you to Ashgrove Medical Practice. Registering with a GP is one of the most important things you can do for your health, and the process is straightforward.
Who Can Register?
Anyone can register with a GP practice in England. You do not need proof of address, immigration status, or identification to register — though providing these can help us process your registration more quickly. You have the right to register with any GP practice, although practices do have defined catchment areas.
How to Register: Step by Step
- Complete a GMS1 form. This is the standard NHS registration form. You can collect one from our reception desk or download it from the NHS website. The form asks for your personal details, previous GP information, and any existing medical conditions.
- Provide identification (if available). While not mandatory, bringing photo ID (passport, driving licence) and proof of address (utility bill, bank statement) helps us verify your details. If you do not have these documents, you can still register.
- Submit your form. Hand your completed form to our reception team. They will process your registration and request your medical records from your previous GP (if applicable).
- Attend a new patient health check. Within a few weeks of registration, our healthcare assistant will invite you for a new patient health check. This includes blood pressure, height, weight, BMI, and a review of your medical history.
What to Expect at Your First Appointment
Your new patient health check is an opportunity for us to get to know you and understand your health needs. The appointment typically takes 20-30 minutes and covers:
- Blood pressure measurement
- Height, weight, and BMI calculation
- Urine sample analysis
- Review of your medical history and current medications
- Discussion of any current health concerns
- Lifestyle assessment (smoking, alcohol, exercise)
- Immunisation status check
"Registering with a GP gives you access to the full range of NHS primary care services, from routine appointments and prescriptions to specialist referrals and health screening programmes."
After Registration
Once registered, you can:
- Book appointments with our GPs, nurses, and healthcare professionals
- Order repeat prescriptions online
- Access your medical records through the NHS App
- Be invited for NHS screening programmes relevant to your age and gender
- Receive vaccinations including annual flu jabs
Frequently Asked Questions
Can I register if I do not have a permanent address?
Yes. If you are homeless or do not have a fixed address, you can use the practice address to register. Everyone is entitled to NHS healthcare regardless of their housing situation.
Can I stay registered with my old GP?
You can remain registered with a practice outside your area, but it may limit access to home visits. We recommend registering with a local practice for the best continuity of care.
How long does it take to transfer my records?
Electronic medical records are usually transferred within 1-2 weeks. Paper records (if any exist) may take longer. You can use our services immediately upon registration — you do not need to wait for records to transfer.
Need Medical Advice?
If you have health concerns, book an appointment with your GP. Do not rely on online information for medical advice.
